If your office is within our servicing area in NE Ohio, our Laboratory courier service will provide in-office pick-ups.

- Mark enclosed items on the Rx form.
- Properly package any small items marked with patient’s name (i.e. implant parts) in a secure box with the case.
Shipping a Case:
Our shipping labels can be forwarded to your office quickly via email or fax.

- Call our office for a shipping label and to arrange a pickup at your location.
Packing Your Case:
Packaging your single and multiple cases:
- Mark all models and impressions with patient’s name and doctor’s initials
- Mark patient’s name and doctor’s initials onaquart sizesealablebag(s) with case items enclosed. Staple Rx form securely to bag(s).
- Always pour alginate impressions in stone and separate from impressionbefore shipping to the lab to avoid distortion. Separate and wrap multiple models to prevent breakage.
- Secure bites, crowns, implant parts or other small items in a small bag or container with patient’s name.
- Pack with bubble wrap or foam;making sure contents are secure and will not shift. Seal box securely with packing tape.
